Home From Hospital Care

When you’ve been in hospital for treatment and aren’t feeling yourself, there is nothing better than the comfort of your own bed. Our carers at Divine Health Services work round the clock to help you settle back into your life at home. Whether you need someone to help establish a routine after a long period in hospital or you need more intensive medical care, our nurses enable you to recover in the comfort of your own home. From arranging transport on the day of discharge to installing any necessary equipment for the best care, we’ll do everything we can to make the transition as simple as possible.

As this is your home and we try to administer our care in the least disruptive way possible so that you can recuperate in peace. Prior to your discharge, we’ll visit you in hospital and, with your permission, assess your home to ensure that everything’s ready for your return. Depending on the treatment you’ve had, you may only need us for a few days following your return home. Whatever your requirements, we will work with you to determine the plan that’s right for you
